Meat and Antibiotics: Getting Our Animals Off Drugs

The Food and Drug Administration (FDA)—gingerly, gingerly—took a step on Monday towards addressing one of the most fundamental but unknown threats to public health: the overuse of antibiotics in animal food and water. The FDA said in a new policy document that the uses of antibiotics for agriculture should be limited to treated sick animals, and that veterinarians should be involved in giving out the drugs. (See the FDA document here.) The change came in part out of concerns that the use of antibiotics on animals was helping to breed resistant bacteria—an estimated 100,000 people a year now die from hospital-acquired bacterial infections that can no longer be treated with most antibiotics, thanks to resistance.

While doing nothing to change the present oversight of antibiotics, the document is the first signal in years that the agency intends to rejoin the battle to crack down on agricultural uses of antibiotics that many infectious disease experts oppose.

Doesn’t sound like a big deal, right? Drugs like penicillin and tetracycline were developed to help sick people, and it might stand to reason that they should be preserved for use in sick people—not to speed the growth of pigs, chicken and cattle. And given the scale of the problem—according to the Union of Concerned Scientists (UCS), about 13 million lbs. or 70% of all antibiotics in the U.S. goes to food animal production—the FDA’s statement was just a baby step. As Deputy FDA Commissioner Joshua Sharfstein told reporters in a news conference after the recommendation was made: “We’re not expecting people to pick up this guidance and change their practice tomorrow. This is the first step in the FDA establishing the principles from which we could then move, if necessary, toward other mechanisms of oversight, which is regulation.” Sharfstein was so cautious he might as well have been wearing a flak jacket and a crash helmet while speaking.

But he has reason to be careful. the powerful agriculture industry has fought every attempt by federal regulators to crack down on the use of antibiotics in animals—and they’ve won every time . The meat industry argues that antibiotic use in animals is far lower than its critics say—the Animal Health Institute, a trade group, estimates that just 13% of all agricultural antibiotics are used for growth promotion, with the rest going to treat sick animals or prevent illness. And they’ll fight hard against any attempt by the government to limit their ability to dispense drugs to the millions of swine, chicken and cattle being raised for food in the U.S. “Show us the science that use of antibiotics in animal production is causing this antibiotic resistance,” Dave Warner of the National Pork Council told the Washington Post.

It is difficult to make the direct connection between an antibiotic being used on a pig in Iowa and a person dying from drug-resistant Salmonella in a hospital in New York. But there’s no disputing that doctors are increasingly worried about the rise of resistant bacteria and the threat the problem poses to each of us. The Centers for Disease Control found in its 2005 annual report that half of all Campylobacter infections are drug resistant. One in five Salmonella infections, and nearly 100,000 Salmonella infections would resist treatment with at least five antibiotics, according to Congressional testimony in 2009 by Margaret Mellon, the director of the food and environment program at the UCS. The World Health Organization has sounded the alarm of antibiotic resistance, and the Institute of Medicine has estimated that longer hospital stays because of resistant food-borne microbes cost the U.S. an additional $4 to $5 billion a year in health care costs—plus all the additional time sick people have to take off work. Superbugs like MRSA—Methicillin resistant Staphylococcus aureus—are truly scary, resistant to nearly every antibiotic we have at our disposal. We’re in an arms race with bacteria—and we’re in danger of losing, as Dr. Ken Harvey of La Trobe University’s School of Public Health in Melbourne told the Australian Broadcasting Corporation today:

“Although we’ve developed new antibiotics, the more we’ve used them the more the bacteria have become resistant. It’s sort of Darwinian selection of the species if you like.” Dr Harvey says bacteria have the edge in this evolutionary battle. “We’re grossly outnumbered by bacteria and they’ve got very efficient mechanisms of mutating, becoming resistant, passing on that mechanism of resistance to other germs.”

And a small but growing number of studies are connecting the use of antibiotics on farms to resistant bacteria in hospitals. A strain of MRSA responsible for 20% of all MRSA infections in the Netherlands has been shown to be transmitted from pigs to farmers and their families, and hospital staff. Even news organizations as mainstream as CBS have begun to raise questions about the connection between antibiotic use in our farms and resistant bacteria, as this Katie Couric investigation from February shows.

The meat industry has argued that restricting antibiotics in animal feed will lead to unbearably high costs, but other countries have already taken that step without destroying their to make bacon. In Europe, Denmark led the way by banning antibiotics for growth promotion in 1998, after concerns grew about resistant bacteria. In the years since the industry has thrived—Danish swine production  from 18.4 million in 1992 to 27.1 million in 2008, while total antibiotic use decline 51% from an all-time high in 1992. I had a chance to see the situation for myself when I was in Denmark last December for the UN climate change summit. Pork farmers told me that the rule was a pain when it was first put into practice, but that they’d adjusted, taking better care of their pigs because they knew that they could no longer rely as heavily on drugs. (Antibiotics were still available for sick animals, but were much more closely regulated.) And in 2006 the European Union followed suit and banned antibiotics for growth promotion.

Given all that, what the FDA did on Monday is too weak, not too strong—as some public health groups argued. “Te FDA has proposed good steps, but they have not gone far enough or moved fast enough,” said Representative Louise Slaughter of New York, who happens to be the only microbiologist in Congress. “We cannot wait any longer.” Given how strong the farm lobby is in the U.S.—which I can tell you from experience—we might be waiting forever.

One explanation has to do with the fact that while women in the United States start their own businesses at roughly twice the rate of men, they are still playing catch-up in most parts of the world. “Women are still at the beginning of their journey to entrepreneurship or higher levels of entrepreneurship,” says Sharon Hadary, the former and founding executive director of the Center for Women’s Business Research. According to that group, only 20 percent of all businesses over $1 million are women-owned enterprises.

Then there are more fundamental issues related to personal goals. It seems that men more often define success in monetary terms while women focus on vision and mission. “Men tend to start businesses to grow them to be large and to be the boss while women start them to do something meaningful and to make a difference,” notes Hadary, who now consults on women’s leadership through her firm Sharon Hadary & Co.

Becoming a billionaire isn’t their objective,” she adds, “As women, many are less likely to have a goal of generating personal wealth, though we are starting to see more and more of that motivation among the younger generation.” Instead they are typically more focused on building companies that reflect their values, provide employment opportunities, do what’s important to them and have the opportunity to fulfill family goals.

Teresa Nelson, McCandless Chair in Entrepreneurship at the Simmons School of Management concurs: “Research shows that women are drawn to organizations that provide values beyond wealth.”

Take the Risk!

“No risk, pay the cost.  Know risk, reap the rewards.”
There’s a two sentence summary of the new book by Dr Ben Carson called “Take the risk.”  We do live in a risk-adverse culture and we put safety above everything.  We’re in the car-buying process and we want to take a pre-owned car in to the mechanic to look it over, and then we start negotiating on the extended warranty.  Dr Carson asks you to consider the following:  “by insulating ourselves from the unknown – the risks of life – we miss the great adventure of living our lives to their full potential.”

Without a doubt, I will say that life is lived fuller when risks are taken! We still don’t know the extent of the devastation that 200,000 gallons a day of crude oil spewing into the Gulf waters will have on our country. So far, it’s proving to be one of the largest human-caused environmental disasters in U.S. history. We know that the effects on the animal and plant populations have been disastrous, and we’ll be watching closely as the they continue to unfold. However, one question remains: Will this oil spill affect our health?

The short answer is, yes. There are well-documented analyses on the effects of environmental pollution of previous oil spills — some which have occurred inland and certainly the Exxon Valdez spill in the Alaskan waters of the Prince William Sound in 1989. You have to remember that it only takes about a quart of crude oil to pollute 150,000 gallons of water. Crude oil contains substances such as benzene and polycyclic aromatic hydrocarbons that have been proven to cause severe reactions in humans — some mild, such as nausea, vomiting and fatigue. But studies have also linked exposure to these compounds to more serious conditions like leukemia and certain types of cancers.

Another negative health effect that could come from this massive environmental catastrophe is the potential for exposure to heavy metals, such as lead — which we know can be very detrimental to the health of an unborn child — resulting in low birth weight, developmental delays, miscarriage and even stillbirth. So pregnant women are especially vulnerable to these heavy metals.

Look, we all know what happens to us humans when our food chain gets contaminated with chemicals and the havoc it can wreak on our health. So the bottom line the best protection we have a situation like this is educating ourselves. The government is doing what they can to clean up and prevent human consumption of contaminated food, but we should also practice personal responsibility for the things we eat. It’s important that attention be paid to the communities in the immediate areas of potential exposure. And certainly, for seafood lovers like myself, be aware of where your food is coming from.

The potential for health problems resulting from airborne pollutants where oil is washing ashore is minimal. But for those living in affected areas living with pulmonary diseases, like asthma or emphysema, aromatic irritants could enhance symptoms. As of now, there haven’t been any reports of respiratory side effects, but just keep an eye on the story and take the proper precautions.

Finally, let us pray for the people of Louisiana and surrounding coastal communities, and for the fisherman whose livelihood will surely be affected by this terrible accident. In an area that has suffered great losses at the hands of natural disasters like Hurricane Katrina, it’s imperative that we continue to show our love and support for the communities that have been a source of joy for Americans.

The Oil spill and your health

Gulf Coast Oil Slick Could Have Impact on Seafood and Air Quality

April 30, 2010 — The Deepwater Horizon incident occurred about 50 miles southeast of Venice, La., on April 22 after an explosion and fire damaged a Transocean oil rig, causing it to burn for hours and sink. There were approximately 700,000 gallons of fuel onboard before the fire, and exactly how much of this fuel burned before it sank is not known, according to the National Oceanic and Atmospheric Administration (NOAA) in Silver Spring, Md.

The spill could affect hundreds of species of fish, birds, and other wildlife along the Gulf Coast, which is one of the world’s richest seafood grounds. According to NOAA, there may be risks for people working as oil spill responders, observers, and in wildlife rehabilitation due to inhalation of fumes.

Oil-derived compounds including polycyclic aromatic hydrocarbons (PAHs) tend to be readily cleared from fish and mammals, so eating fish after an oil spill generally does not pose a health risk to humans. But oysters, shrimp, and crab do not readily clear PAHs and are more likely to accumulate these potentially toxic substances after oil exposure, NOAA states.

These compounds can also be toxic for fish eggs and larvae and may can cause a wide range of health problems in other marine species.

Impact on Air Quality

LuAnn White, PhD, a professor of environmental health sciences and the director of the Tulane Center for Applied Environmental Public Health in New Orleans, is on the front lines of the Deepwater Horizon oil spill.

“We are watching it very closely,” she tells WebMD. “The odor from the spill could affect people with respiratory diseases such as asthma and emphysema, but the spill happened off shore, so that is good because there is no one out there,” she says. The occupational health issues are immense, but proper safeguards are being taken by oil spill responders and others involved in the clean-up process, she adds.

The oil involved in the spill was sweet crude oil. “It didn’t have sulfur, so it doesn’t smell as bad as other types of oil and it is composed of lighter compounds that will evaporate,” she explains.

So far, she says, the odor is light and transient.

There has been other reassuring news regarding air quality, White tells WebMD. “There has been air monitoring along the coastline and that didn’t pick up anything so far.”

Touching the oil can cause skin irritation and burning, she says. “Standing next to it and not touching it will not cause any problems.”

As far as drinking water concerns, there are none. “The Gulf of Mexico is a saltwater body, not a freshwater source, so drinking water contamination is not an issue,” she says.

Seafood Contamination

The risk of contaminated seafood reaching consumers is also relatively low, she says. “Fisherman won’t be allowed to collect seafood in any area with an oil spill, so seafood caught will come from other areas.”

Recall: Lettuce

Did you see the news about how Lettuce is now recalled due to a strain of E. Coli in pre-shredded Romaine?  At least 30 people in four states got sick, 12 went into the hospital and 3 had kidney failure!  The outbreaks happened in Michigan, Ohio, New York, and Tennessee… all from a farm in Arizona.  So not only was that lettuce shipped over thousands of miles, it was contaminated.

E. coli Outbreak From Romaine Lettuce Widens

May 13, 2010 — At least 30 people in four states have been sickened by a rare, virulent strain of E. coli in pre-shredded Romaine lettuce.

Twelve of the victims have been hospitalized, three with kidney failure. There have been no deaths to date. Some cases may have gone unreported because many labs do not test for the E. coli strain causing the outbreak. Michigan, Ohio, New York, and Tennessee have reported cases.

The contaminated Romaine lettuce was sold by restaurants and by grocery/deli salad bars and processed by Freshway Foods in Sidney, Ohio. An unopened bag of Freshway shredded Romaine lettuce was found to carry the E. coli with the same genetic fingerprints as E. coli isolated from patients.

Freshway has recalled all Romaine lettuce with a use-by date of May 12 or earlier.

Freshway got the lettuce from a farm in Yuma, Ariz. Another lettuce distributor, Vaughan Foods, has also recalled lettuce from the same farm. However, no illnesses have yet been traced to lettuce distributed by Vaughan.

Lettuce harvested from other areas does not appear to be associated with the outbreak, the FDA investigation suggests.

The illnesses occurred between April 10 and April 26. Because of a lag time between the last reported illness and testing, it’s possible new illnesses may still be occurring.

People with E. coli infection usually get diarrhea and abdominal cramps for two to eight days after eating a contaminated food. Most people recover within a week, but some cases are much more severe and last much longer.

Some people may develop a form of kidney failure called hemolytic uremic syndrome or HUS. HUS tends to appear just as the diarrhea is getting better. Symptoms may include fever, abdominal pain, pale skin, fatigue, irritability, decreased urine output, unexplained bruises or bleeding from the nose and mouth, and swelling of the face, hands, feet, or body.

According to World News April 20, 2010: More and more male fish in the Potomac River are exhibiting female traits, but the reasons for the growing number of intersex fish are not known.

The Potomac Conservancy says more than 80 percent of the river’s male fish are producing eggs or exhibiting other female characteristics.

The Potomac River supplies much of the region’s drinking water, and that has the group calling on Congress to solve the fish mystery.

Scientists say chemicals that mimic hormones and disrupt the endocrine system are believed to be the leading reason for the intersex conditions.

The chemicals may be birth control pills, pharmaceuticals or some of the newer herbicides and pesticides that are flushed into wastewater or that run off farms and feedlots.

Rep. Jim Moran, D-Va., is sponsoring a bill to study intersex fish problems. He says he wants to know more about the chemicals.

“Why they are so prevalent in the water? Why they are causing these fish with both sexual organs and if there isn’t some kind of connection with human diseases,” he says.

The Potomac River isn’t the only body of water where the problems have surfaced. A U.S. Geological Survey found intersex fish in about one third of 111 sites tested in 2009.

For the past four hundred years, virtually all practitioners of the dismal science we call economics have agreed on one basic premise: namely that a society’s wealth is determined by its supply of physical resources–its land, labor, minerals, water, and so on. And underlying this premise has been another, even more profound, assumption–one supposedly so obvious that it is rarely mentioned: namely, that the entire world contains a limited amount of these physical resources.
This means, from an economic point of view, that life is what the mathematicians call a zero-sum game. After all, if there are only limited resources, one person’s gain must be another person’s loss; the richer one person is, the poorer his neighbors must be.
Over the centuries, this view of the world has been responsible for innumerable wars, revolutions, political movements, government policies, business strategies, and possibly a religion or two.
Once upon a time, it may even have been true. But not anymore.
Whether or not we ever did, today we do not live in a resource-scarce environment. That may seem hard to believe, but the businessperson and the
politician–as well as the butcher, the baker, and the candlestick maker–who
continue to behave as if they were operating in the old zero-sum world will soon find themselves eclipsed by those who recognize the new realities and
react accordingly.
What are these new realities? To put it simply, we live today in a world of
effectively unlimited resources–a world of unlimited wealth. In short, we live in what one might call a new Alchemic world.

There are plenty of confusing topics in nutrition, but fats may take the cake. Are saturated fats like butter and animal fat terribly harmful? Should you worry about whether you’re eating too much of one kind of polyunsaturated fat and not enough of another? What about olive oil? And shouldn’t we be eating as little fat as possible, since so many of us are, well, fat? The distinctions are “enormously confusing unless you’re a lipid biologist,” says Marion Nestle, a professor of nutrition at New York University. There’s a way to translate it all into relatively simple eating advice–more on that later. But first, here’s the skinny on fat.

First, toss out the notion that the lower the fat content in your diet, the better. A certain amount of fat is essential to your body’s functioning. And as you’ve probably heard, all fats are not alike in their effects on blood cholesterol levels, which can affect heart disease risk. Saturated fat, for example, generally increases levels of LDL, or “bad” cholesterol. But while this information was known when the surgeon general issued the first report on nutrition and health in 1988 and the National Academy of Sciences issued its own report in 1989, public health authorities felt that a message to reduce total fat would be best understood by the public. The thought was, says Nestle (who was managing editor of the 1988 report), that since saturated fats from meat and dairy products were the main sources of fat in the American diet, lowering total fat would automatically reduce consumption of saturated fat. That’s certainly true, in theory.

But here’s the rub in practice: “If you take out saturated fat, and you assume someone is in energy balance [i.e. their total calories haven't changed], what do you put in?” says Alice Lichtenstein, a nutritional biochemist and director of the cardiovascular nutrition laboratory at the Jean Mayer USDA Human Nutrition Research Center on Aging at Tufts University in Boston. As it turns out, what you substitute for saturated fat seems to be important. “People reduced fat and replaced that fat with simple carbohydrates and refined sugar,” says Mimi Guarneri, cardiologist and medical director of the Scripps Center for Integrative Medicine in La Jolla, Calif.

That switch was aided by food companies, with their lines of “low-fat,” yet sugar-filled products. A review published online in January in the American Journal of Clinical Nutrition said that research actually shows that subbing in a higher carbohydrate intake, especially refined carbs like white bread and sugary cereals, can actually “exacerbate” blood cholesterol problems, including elevated triglycerides and reduced HDL (“good”) cholesterol levels. To improve cholesterol, people should focus on limiting those refined carbs and reducing excess body weight, the review said. So much for unlimited SnackWells.

By contrast, trading saturated fats for polyunsaturated fats–the omega-3 fatty acids found in certain fish and the omega-6 fatty acids in vegetable oils such as safflower and soybean oils–does seem to offer a heart benefit. An analysis of existing research published in March in PLoS Medicine found that consuming those polyunsaturated fats instead of saturated fats reduces the incidence of heart attacks and cardiac death. Based on the evidence, “polyunsaturated fats are the best…fat to be increasing in the diet,” says Dariush Mozaffarian, an author of the study and an assistant professor at the Harvard School of Medicine and co-director of the program in cardiovascular epidemiology at Brigham and Women’s Hospital.

Ronald Krauss, senior scientist and director of atherosclerosis research at Children’s Hospital Oakland Research Institute, and an author of the ACJN review, agrees. In fact, he says, it’s unclear whether the benefits to cardiac health spring from removing the sat fat or adding the poly. But isn’t saturated fat a “bad” fat? Possibly, but his research, including a meta-analysis also published in the ACJN, found that despite its effects on LDL cholesterol, there’s no evidence dietary saturated fat alone is tied to the risk of coronary heart disease or cardiovascular disease. He also says that even in trials where saturated fats were swapped out in favor of polyunsaturated fats, there was not much benefit in reducing saturated fats below about 9 percent of calories. It may be that saturated fats aren’t as evil as has been thought, but simply allow atherosclerosis to happen without conferring any protection; they raise both LDL and HDL cholesterols, leaving the ratio of the two little changed. By contrast, polyunsaturated fats “move the numbers in the right direction,” says Krauss.

But polyunsaturated fats aren’t free of controversy: namely, whether people should be worried about a too-high ratio of omega-6 fatty acids to omega-3 fatty acids in the diet. Omega-3 fatty acids, found in albacore tuna, sardines, walnuts, and tofu, are linked to improvement in some measures of heart-disease risk; omega-6 fatty acids, found in eggs, certain fish, and safflower and soybean oils, may cause inflammation. At one point the ratio of omega-6s to omega-3s in the American diet was about 2:1; now experts say it might be as high as 40:1 or even 50:1. But there’s no good evidence that they can actually cause clinical harm in humans, says Lichtenstein, who suggests people not fixate on the ratio.

What about olive oil, sesame oil, canola oil, and other oils high in monounsaturated fats? While they’re a good replacement for saturated fats, they don’t seem to have the same level of benefits as do polyunsaturated fats, says Mozaffarian. Most of the evidence of benefit has come from observations about the so-called Mediterranean Diet, but the health-conferring element in that eating pattern hasn’t really been nailed down, he says. And if olive oil itself is beneficial, it may not be the monounsaturated fat it contains, but the phytochemicals, But again, Lichtenstein urges people not to get too hung up on that point. Instead, focus on unsaturated fats, period. If you want to cook some things with soybean oil, stir-fry veggies with sesame oil, and make salad dressing with olive oil, that’s fine.

And that brings us to the easy guideline for consuming fat in the most healthful way possible: Think about foods, rather than chemistry, when you head to the supermarket. The fats in food are all mixtures, and you’ll drive yourself crazy attempting to figure out exact proportions, notes Nestle. Besides, fussing over components ignores the way people actually eat. “I’ve never known anyone to sit down to a tub of saturated fat,” says John La Puma, an internist and author of ChefMD’s Big Book of Culinary Medicine: A Food Lover’s Road Map to Losing Weight, Preventing Disease, and Getting Really Healthy. “People eat food, not saturated fat or trans fat or starch or sugar.”

Lichtenstein agrees, saying we should focus on the pattern of eating, not any single dietary component. And research is fairly consistent on what constitutes a healthful eating pattern: a varied diet that’s heavy on the fruits and vegetables, emphasizes whole grains rather than processed ones, focuses on fish and a limited amount of lean meats and lowfat dairy, and includes liquid oils for food preparation. Following that sort of pattern will naturally lead you to “good” sources of fat, and will reduce levels of the not-so-good–including most trans fats, which are still considered the worst of the bunch. (Hydrogenated oils “have nothing but adverse effects,” says Krauss.)

And of course, remember that the quantity, not just the quality, of your food is important, says Nestle. Rather than having his patients count calories, La Puma recommends foods that will make them feel satisfied, gives them smaller plates, advises against skipping breakfast if it makes them ravenous later on, and suggests they try to sit down when they eat. “It reinforces the feeling of eating as something deliberate rather than just grazing,” he says. But even healthy eaters should focus on portion control. “If you eat a lot of nuts, avocados, or olive oil, you’ll get fat,” says Guarneri. And once it’s around your middle, it’s definitely no longer “good” fat.

The omega-3 fat DHA (docosahexaenoic acid) may alter the function of the brain associated with working memory, according to results of a new study with healthy boys.

Scientists showed, for the first time using neuro-imaging, that supplementation with DHA alters the functional activity in cortical attention networks in humans.

The study follows hot on the heels of backing from the European Food Safety Authority (EFSA) for DHA-related brain and eye health claims for infants. EFSA’s Panel on Dietetic Products, Nutrition and Allergies (NDA) said DHA levels of 100 mg of per day were appropriate for 7-24 month-old infants, along with 200 mg per day for pregnant and lactating women.

Additionally, supplements of omega-3 fats, vitamins and minerals for prisoners may reduce the number of violent and aggressive episodes, according to another recent study.

Nutritional supplements containing vitamins, minerals, omega-3 and omega-6 fats were associated with a 34 percent reduction in violent incidents, according to findings of a randomized, double-blind, placebo controlled trial with over 200 young adult offenders.

Meanwhile, there was a 14 percent increase in the number of reported violent incidents in participants in a placebo group.
